Description
I am an Independant adult social worker. I provide a full person centred and professional service to self funders. I am able to provide, 1) Care Act assessment, 2) Mental Capacity assessment for Lasting Power of Attorney activation and advice 3) Mental capacity assessment for Deputyship applications 4) Completion of Form COP3 - Part B for Deputyship applications 5) Lasting Power of Attorney Certificate provider for LPOA applications 6) Care home sourcing, which includes Residential and Nursing home 7) Care at home sourcing, such as domicillary care and live in care 8) Private hospital discharge planning I also work with legal firms to complete formal Mental Capacity Assessments when they are completing your lasting power of attorney or deputyship application, as well as, Form COP3 - Part B or COP24 Statements. I am able to work across Somerset, North Somerset, Devon, Wiltshire, Berkshire and Hampshire.
